Audio icon/poster images change in Preview/export to ePub

hey - first time posting here... hoping can me. i've searched quite few forums , haven't seen exact issue come yet. apologize if has , didn't find it.

 

i have indd file exporting epub want incorporate audio files. have placed media file (mp3) using standard poster image. in indd file appears show correctly, however, when preview and/or export epub fixed width or reflowable, image changes solid rectangle play button.

all i'd have own image ability click start , stop audio. i've tried custom image, run same problem. interestingly, in ibooks preview after exporting epub, icons show correctly in thumbnail preview, once open file shoes solid rectangular bars:

Animate - problem with duplicating scripts after loop

Illustrator CS6 Ocurrío un error E/S en el archivo.

este error me me sale cada ves que guardo un archivo que ya estado trabajando varias veces, sale cuando ya esta finalizando el proceso de guardar, ya probe cambiar nombre del archivo, ubicación de guardado, convertí curvas las fuentes y sigue saliendo el mismo error, no se que mas hacer, si alguien le ha sucedido y sabe como solucionarlo por favor ayuda. versión exacta (acerca de illustrator) de illustrator? versión sistema operativo?   guardas en el disco duro principal del ordenador o en otro disco, memoria pendrive,...? ese disco que clase de disco, modelo, antiguedad, espacio libre,...?
