i recommend using static livecycle pdf forms version of adobe acrobat includes livecycle. livecycle xfa forms have image field can perform requirements need. once end-user clicks on image field in adobe reader, form brings os file browse dialog prompt import image (jpg, pdf, png, gif) xfa form's image field place holder. image's file bytes encoded base64 string , stored in xdp data of xfa form. xdp data can emailed fdf acrobat pdf forms.

note: when creating xfa forms, should save "static pdf" instead of "dynamic pdf"; unless need features dynamic pdf forms. static xfa forms have limited compatibility software; such itextsharp parsing , merging, dynamic xfa forms proprietary adobe software.
