Collect data from a PDF form Using Adobe Acrobat Pro DC and upload to url

what need collect data form created in adobe acrobat pro dc. creating form straight forward , getting data mailed or tracked through software simple that's not need. need data in form of fdf or html , fetch minimal user interaction. below sample of form shall using:

while adobe provides option of export url, there aren't lot of tutorials online show such process clearly. can see option provided in export below:

if has ever executed this, please guide me through it.

Animate - problem with duplicating scripts after loop

Illustrator CS6 Ocurrío un error E/S en el archivo.

este error me me sale cada ves que guardo un archivo que ya estado trabajando varias veces, sale cuando ya esta finalizando el proceso de guardar, ya probe cambiar nombre del archivo, ubicación de guardado, convertí curvas las fuentes y sigue saliendo el mismo error, no se que mas hacer, si alguien le ha sucedido y sabe como solucionarlo por favor ayuda. versión exacta (acerca de illustrator) de illustrator? versión sistema operativo?   guardas en el disco duro principal del ordenador o en otro disco, memoria pendrive,...? ese disco que clase de disco, modelo, antiguedad, espacio libre,...?
